Freedom of Choice cares for seniors, elders with dementia/Alzheimer, individuals with physical or developmental challenges, homebound, short or long term hospital stays, stroke or heart attack survivors and folks with other debilitating conditions.
Our services include, but not limited to personal care, companion care, homemaker/chore aide, errand/appointments. Our Specialized Care Services include, but not limited to the care of clients with Alzheimer’s & Dementia,
End-of-Life care, Respite, Post-Hospital care, Hospice assistance.
We will meet you where you are, in your home, adult children's home, an apartment, house, a friend or relative’s home, a personal care home, assisted living facility or even in a hospital, we will come to you and provide the care you need.
FOC has limited skilled nursing services for the medically frail/compromised. Should more involved nursing care be needed, our professional client care director will be happy to work with you to provide referrals to nursing services by independent contractors who do provide those necessary skilled nursing services.
Caregivers
At Freedom of Choice, we understand that every individual's needs are unique. We are dedicated to providing you with personalized care solutions tailored specifically to your situation. Our team is focused on your care, comfort and making sure you receive the assistance you need in an a safe, clean and supportive environment.
Each caregiver is carefully screened for national criminal background check, they must submit a current TB skin test, current CPR & Basic First Aid certificate, DL, Social Security card and a signed application with a 5 year work history.
All employees credentials, verification forms and other documents remain in a locked file at our corporate office and can be reviewed by our clients and/or family at any time. Rest assured, FOC caregivers are carefully selected, closely supervised, insured and well-trained. Most of all, they are honest, friendly and reliable.
Personal Care
Assistance with ADL: eat, bathe, mouth/dental, toileting, grooming, transfers, medication reminders, basic skin care, dressing, assist with prosthetics, taking vitals and taking care of other housekeeping tasks.

Provide companionship, friendship and emotional support. Stimulating conversation, listening, sharing experiences, activities, games, reading newspaper, magazines or story books. Help keep client in contact with family/friends & outside world. Accompany clients to recreational or social events, escort to planned visits/outings/walks. Writing or typing letters, organizing & reading mail. Medication reminders, assist with pet care and providing a safe environment.

Companion Care
Live In or
24 Hour Care
Live-in home care means that a caregiver lives in your home, either longer-term or for a short period, to help with anything from assisting with personal care needs, providing companionship, reading the newspaper/mail, assist with writing letters to friends & family, assist with paying bills and provide housekeeping task.
Live-in caregivers are entitled to 8 hours of sleep, so if the needs are too great to accommodate sleep, you might choose traditional 24-hour care (around the clock care). In this case, a caregiver is awake at all times. This 24-hour traditional care means that several caregivers rotate shifts (typically 8 hours at a time) therefore someone is always awake and alert to help your loved one.
